Meeting the person going into care in a relaxed environment gives me the opportunity to get to know them, understand how they are feeling and what their expectations are about moving into residential care. I will be able to reassure them and discuss any fears they may have.

Documentation
We will discuss the aspects of care that is required and the relevant paper work that will need to be completed. I can also simplify the process by assisting you to complete the paper work.

Residential Care:
Residential Care caters for people requiring Low, High care needs and specialized Dementia Care.
To help identify the best care facility that will meet you and your loved one’s expectations we will also discuss aspects such as: Accommodation type; single or shared room, with or without ensuite.
The location of the care facility with consideration to ease of commute for family visits.
Residential Care Facilities:
Following our initial consultation I will develop a short list of five facilities that will best suit you and your loved one’s needs. I will visit each facility and provide you with an overview on how well each facility aligns with your preferred options and expectations. Consideration will be given to matching care requirements, social aspects and geographical preferences for the convenience of both your loved one and your family.
If you would like me to accompany you when you visit any of the short listed facilities I am happy to do so at a mutually agreed time.
When you have made a decision on the care facility of your choice I can assist with the lodgement of the documentation.
Social History
To help ease the transition into care I can assist with the compilation of a brief social history book. This book lists your loved one’s likes and dislikes. This will assist the staff in reducing any anxiety your loved one may be experiencing with their move into care and can help them feel welcomed and comfortable.
I will be with your family every step of the way to help support you and your loved one during this sensitive time.
